Due to recent changes in some Medicare requirements, practitioners who are members of the Covenant SurgiCenter medical staff in addition to the staffs of Covenant Medical Center and/or Covenant Children’s Hospital will be required to complete duplicate credentialing forms. These forms will be designed specifically for the SurgiCenter.
